As a patient of Edina Plastic Surgery, you have the right to:

• Exercise your rights without regard to sexual, cultural, economic, educational, or religious background or source of payment.
  
• Considerate and respectful care.
  
• Know the name of the physician who has primary responsibility for coordinating your care and the names and professional relationships of other physicians who will see you.
  
• Receive information from your physician about your illness, course of treatment and prospects for recovery in terms that you can understand.
  
• Receive as much information about any proposed treatment or procedure as you may need in order to give informed consent. Except in emergencies, this information shall include a description of the procedure or treatment, the medically significant risks involved in each and the name of the person who will carry out the procedure or treatment.
  
• Participate actively in decisions regarding your medical care. To the extent permitted by law, this includes the right to refuse treatment.
  
• Full consideration of privacy concerning your medical care program. Case discussion, consultation, examination, and treatment are confidential and should be conducted discreetly. You have the right to know the reason for the presence of any individual.
  
• Confidential treatment of all communications and records pertaining to your care and your stay at the center. Your written permission shall be obtained before your medical records can be made available to anyone not directly concerned with your care.
  
• Reasonable responses to any reasonable requests you may make for service.
  
• Leave the center, even against the advice of your physicians.
  
• Reasonable continuity of care, including knowing in advance the time and location of appointments as well as the physician providing the care.
  
• Be advised if Edina Plastic Surgery or your surgeon is engaged in human experimentation affecting your care or treatment. You have the right to refuse to participate in such research projects.
  
• Be informed by your physician or a delegate of your physician of your continuing health care requirements following your discharge.
  
• Examine and receive an explanation of your bill, regardless of source of payment.
  
• Know which rules and policies apply to your conduct as a patient.
  
• Have all patients' rights apply to the person who may have legal responsibility to make decisions regarding medical care on your behalf.

Patient responsibilites

Your responsibilities at Edina Surgery Center are to:

• Provide correct information on the patient registration form.
  
• Provide the nursing staff with a complete health history – including allergies, surgical history and current medications.
  
• Follow preparation instructions and to call with any questions or problems.
  
• Bring a responsible adult driver as requested.
  
• Follow the physician's post-procedural instructions.

Dos and don'ts

To ensure that your surgery and recovery go smoothly, it is imperative that you follow these simple instructions:

• Do not eat or drink anything (including water) after midnight on the morning of your surgery.
  
• Do not take Advil, Motrin, Ibruprofen, Vitamin E, Lecithin, Nuprin, aspirin, or any aspirin-containing compounds for two weeks before and after your operation. This includes Bufferin, Anacin, Excedrin, Vanquish, Alka Seltzer, Pepto-Bismol, Aleve, etc.
• You should use only Tylenol to relieve any minor pain.
  
• Discontinue smoking. Cease Nicorette gum and Nicotine Patch for the same amount of time.
  
• If you experience any health changes between your most recent visit to Edina Plastic Surgery and the day of your surgery, please notify your surgeon.
  
• Someone must accompany you to the surgery center and be properly instructed on when to pick you up after your surgery.
  
• As a safety precaution, ou may not drive yourself home.
